Transfer students from other Catholic schools
Transfer students from public schools
Kindergarten students MUST be five years old by September 1st of the year that they start school. At registration, please provide a copy of the student's county birth certificate, and your child’s St. Mary Nativity Parish baptismal date (if your child was baptized elsewhere, please provide a copy of the baptismal certificate).
Physicals & Dental
All students entering Kindergarten must turn in a signed, current physical, as well as an Illinois Department of Public Health dental form. Please call your physicians early for these appointments; please don't forget to make copies for your records.

We look forward to welcoming you to St. Mary Nativity
