We ask that you turn in a copy of the contract signed with your registration fee ($200 per family). We accept cash, checks, money orders, and debit/credit cards. You can drop off, mail, or email the contract with the payment. You will have the option to pay at the end of the registration process, or if you would like to pay over the phone, you can call 815-886-5953 x420. 

 

New families, please bring a certified birth certificate so that we can make a copy for our files. All students need a copy of their birth certificate on file. Some students also need physical/immunization records as well as eye and dental exam forms.

• Preschool is required to have a Physical/Immunizations form.
• Kindergarten is required to have a Physical/Immunizations form, an eye exam form, and a dental exam form. 
• Second grade must have a dental exam.
• Sixth grade is required to have a Physical/Immunizations form and a dental form.

Your child(ren) will only be added to their class list once we have all the required documents and payment. The office is open Monday-Friday from 8 a.m. to 2 p.m.  on all regular school days.
